Guidelines for antenatal care vary from person to person. The NICE guidelines recommend up to 10 antenatal visits for the first pregnancy and 7 visits for subsequent pregnancies. The World Health Organization (WHO) guidelines generally recommend 8 antenatal visits, although this number is not strictly fixed and depends on everyone’s pregnancy status. For Cambodia’s national protocol, low-risk pregnancies are recommended to have 4 antenatal visits, while medium- and high-risk pregnancies are recommended to have at least 8 visits. Below, you will understand the phased monitoring for 8 antenatal visits.

Early pregnancy: Best is before 12 weeks

  • Comprehensive initial health assessment
  • Medical/Obstetric/Family History
  • Blood pressure, weight, and other baseline measurements
  • Blood tests and screening for infections: Complete blood count, blood group/Rhesus, and screening for sexually transmitted infections (HIV, syphilis, and hepatitis B)
  • Proteinuria and glycosuria
  • Nutrition and lifestyle counseling
  • Discussing medications, supplements, and risk factors
  • Early ultrasound examination

20-22 weeks:

  • Continue to monitor the health of the mother and baby (fundal height, general examination)
  • Proteinuria and glycosuria
  • Tetanus vaccination
  • Continued use of supplements, prevention, and counseling
  • Ultrasound scan/anomaly scan, in accordance with guidelines

26 weeks:

  • Continued assessment of the mother and the fetus
  • Monitor blood pressure, weight, and the fetus’s growth
  • Glucose tolerance test (screening for gestational diabetes)
  • Check for complications of pregnancy
  • Continue preventative care and counseling

28-30 weeks:

  • Monitor the well-being of mother and child
  • Assess the position of the fetus in the womb
  • Discuss the danger signs

32-34 weeks:

  • Continue monitoring the health of the mother and baby (fundal height, general examination, heart rate, fetal movement, BMI)
  • Complete blood count (CBC)
  • Proteinuria and glycosuria
  • Danger signs, breastfeeding, newborn care

36 weeks:

  • Mother and child monitoring, similar to the 36th week, as well as guidance on the birth plan, the importance and scheduled time for postpartum check-ups, and postpartum danger signs.

38 weeks:

  • The examination is similar to week 38 but includes checks for the baby’s position, weight, amniotic fluid volume, and placenta location, as well as newborn care and nutrition.

40 weeks:

  • General check-ups such as continuous monitoring of maternal and fetal health (uterine height, general examination, heart rate, fetal movement, and fetal position in the womb), as well as danger signs (especially monitoring fetal movement in the womb)

Danger signs or when to seek immediate medical attention
