In our previous blog “Understanding Sexually Transmitted Infections (STIs): Symptoms, Prevention, Testing and When to See a Healthcare Provider”, we learnt about what STIs or sexually transmitted infections are, the concern of rising STIs prevalence amongst Cambodian youths, and most importantly, how to spot these STIs and get treatment early for an uncomplicated recovery. Now, the problem is, how do you spot an STI if the symptoms are silent? In such cases, you must assess if you need to get tested or not. Do not worry; we can help guide you through this.
What exactly are silent STIs?
Silent STIs are sexually transmitted infections that often do not show clear signs or symptoms to alert you. Another way to call them is asymptomatic STIs.
Depending on the population studied by WHO, those with chlamydia and gonorrhea infections ranged from 13% to up to 99% amongst different populations and have no symptoms. In the United States, 50% of people assigned male at birth and up to 70% of those assigned female at birth have chlamydia without symptoms.
Other common asymptomatic STIs include, but are not limited to, herpes simplex virus (HSV), and human papillomavirus (HPV). HIV is also amongst the STIs that remain asymptomatic and silent until it reaches a later stage.
The burden and dangers of silent STIs
The dangers of asymptomatic STIs are at a high chance of them spreading to another person without them knowing. Those who are unaware of themselves having STIs may be more inclined to have unprotected intercourse with multiple partners, leading to a continuous spread of the infection, who will then infect their other partners.
In addition, those that have asymptomatic STIs may not be able to get timely treatment, leading to a more complicated course of treatment. For example, in cases of those with unknown HPV or human papillomavirus infection, waiting until obvious signs may lead to a more serious case.
Those with asymptomatic STIs also face a higher risk of a complicated pregnancy, infertility, a higher risk of cancer, and susceptibility to HIV infection.
When to get tested and how the tests are done
One way to protect yourself and your partner is knowing who should get tested and in what situations you should get tested or screened for STIs. Here’s when you should consider getting tested:
- If you’ve had sex without protection, whether vaginal, anal, or oral.
- If you’ve used a shared sex toy.
- You should wait 2-3 weeks to get tested after having unprotected sex.
- Other important groups that should consider regular screening include men who have sex with men, pregnant people, people living with HIV, adolescents and young people (<25 years), and sex workers with access to healthcare. For more comprehensive information on when or how to screen for STIs, here are the full CDC recommendations.
As stated in the beginning, STIs are incredibly common. Some may harbor an STI without even knowing. Therefore, getting treatment on time is the best way towards a non-complicated recovery. To get timely treatment, you need to understand that there is no shame with living with STIs and be willing to step up and protect your partners by having protected intercourse and getting tested.

FAQs:
- What are silent STIs?
Answer: Silent STIs, also known as asymptomatic STIs, are sexually transmitted infections that do not show clear signs or symptoms. This means a person can have an infection without realizing it and may unknowingly pass it on to others.
- How do you spot a silent STI?
Answer: Silent STIs cannot be identified by symptoms alone because they often cause no noticeable signs. The only reliable way to know if you have a silent STI is to get tested, especially if you think you may have been exposed or are at higher risk.
- When should you get tested for STIs?
Answer: You should consider getting tested if you have had unprotected vaginal, anal, or oral sex, or if you have shared a sex toy. Regular screening is also important for groups such as men who have sex with men, pregnant people, people living with HIV, adolescents and young people under 25 years, and sex workers who have access to healthcare.
- Are STIs a problem if you have no symptoms?
Answer: Yes. Even without symptoms, STIs can still be passed on to sexual partners and may lead to more serious health problems if left untreated. Early testing and treatment can help prevent complications and protect both your health and your partner’s.
